ABSTRACT

Role of Radiation Therapy in Early Breast Cancer

Breast conserving therapy (BCT) is the accepted method of treatment for breast cancer worldwide. BCT is possible in early breast cancer including DCIS. It has been demonstrated in many randomized clinical trial that survival with BCT is equivalent to mastectomy.

The 20-year update of NSABP B-06 trial reported 39.2% local failure after lumpectomy alone compare to 14.3% with lumpectomy and irradiation so the use of whole breast irradiation with or without a boost is the standard of care for BCT at the present time.

Regarding post-operative radiotherapy in early breast cancer, radiotherapy has a firm role in this situation because the risk of local-regional recurrence after mastectomy is related to the number of axillary lymph nodes involvement. Post mastectomy irradiation of the chest wall reduces the risk of local failure. The NCCN guideline suggested post-operative irradiation in 1-3 axillary nodes positive and recommended irradiation to the chest wall if 4 or more axillary lymph nodes positive.

The result of our patients who were treated with BCT will also be presented.
